Position: Case Manager for Russian Speakers

Required Education, Knowledge and Experience:

  • Bachelors in social work or related field required, Master’s Social Worker preferred
  • Fluent in Russian and English
  • Prior case management experience with older adults
  • Strong assessment, individual planning, and helping skills
  • Knowledge of and/or experience with case management and care coordination approaches with older adults
  • Knowledge of and/or experience with older adult community resources
  • Computer skills including Microsoft Word, Excel, Outlook, and client database
  • Strong organizational, oral, and written skills
  • Works well independently and in a team

Position Description:

 

Part time position providing case management services to Russian Speaking Jewish older adults and some individuals who qualify for services due to disability.  Provide comprehensive assessment, individual planning, and provision of case management and care coordination to older adults.  Provide care in coordination with healthcare providers, caregivers, family members and others as appropriate.  Additionally, includes outreach, translation and interpreting services, and other services and activities for the broader Russian speaking community.   Requires ability and willingness to travel and provide community-based services, primarily in Johnson County, KS and Jackson County, MO.  Work independently and as part of a team. Develop collaborative relationships with older adult serving organizations and individuals in the community.

Essential Responsibilities:

  • Complete program intake and eligibility determination.
  • Complete home or community-based needs and financial assessment for older adults.
  • Develop a written individual plan appropriate to the needs of the person served.
  • Provide bi-lingual community based case management and care coordination services to older adults and/or their caregivers.
  • Coordinate care with health care providers, caregivers, family members and other community and agency resources as appropriate.
  • Outreach, translation and interpretation services.
  • Provide information and referral as needed.
  • Complete periodic financial reassessments.
  • Document all service transactions in accordance with agency procedure and policy.
  • Assist with agency Helpline as needed or requested.
  • Other duties as assigned.
