In-Home Older Adult Therapist
- PT 20 HRS/WK
- Work location in both KS & MO

Position Description:
Two-year grant funded position providing in-home counseling to older adults dealing with depression and co-occurring conditions. Requires ability and willingness to travel by car and provide home-based treatment services in Johnson County, KS and Jackson County, MO. Provide comprehensive assessment, treatment planning, and provision of evidence-based treatment approaches of late-life depression and co-occurring conditions. May include some additional office-based outpatient therapy. Provide care in coordination with healthcare providers, caregivers, and others. Work independently and as part of a team of therapists, case managers, and others. Promote and market the program and develop collaborative relationships with older adult serving organizations and individuals in the community.
Essential Responsibilities:
- Complete home-based comprehensive assessment and treatment planning for older adults dealing with depression and co-occurring conditions.
- Provide in-home counseling services to older adults and/or their caregivers.
- Coordinate care with health care providers, caregivers, case managers and others as appropriate.
- Document all clinical transactions in accordance with agency procedure and policy.
- Collect and document outcome and other performance data per grant description.
- Complete interim grant reports in coordination with Clinical Director.
- Provide caregiver support group
- Provide some office-based outpatient therapy as caseload permits
- After-hours on-call on rotation.
- Market and promote the in-home program through direct and other contact with older adult serving groups and organizations, health care providers, and others as appropriate.

Requirements
- Licensed masters level mental health professional (dually licensed in MO and KS), Clinical Social Worker preferred
- Prior mental health experience with older adults
- Strong assessment and treatment planning skills
- Knowledge of and/or experience with evidence based- treatment approaches for older adults with depression
- Ability to market program (including public speaking) to older adult serving organizations, healthcare providers, and others
- Computer skills including Microsoft Word, Excel, Outlook, and client database
- Strong organizational, oral, and written skills
- Works well independently and in a team
- Must have own reliable transportation and proof of insurance
