Older Adult Assessments

As the population is ages, people are living longer with many wanting to maintain independent lives. In order to maintain a safe and healthy quality of life, many older adults might need modifications in their homes. A variety of other issues also arise concerning how the older adult manages the tasks of community living into old-age.  The problem is that far too many families do not know where to turn to help find the answers to some of these problems.

Jewish Family Services has developed a comprehensive three-tier Older Adult Assessment to help individuals and their families find the answers they need. The assessment is conducted by a team of professionals in mental health, nursing, dietary, and occupational therapy.  All assessments are provided in cooperation with the family as much as is appropriate.

The Older Adult Assessment provides the individual and/or family with a comprehensive look at the current status of the older adult’s community living abilities with an eye toward what would help the person age in place as successfully as possible. A written report covering each stage of the assessment, complete with a summary, recommendations, and referrals, is provided to the family.

Tier 1: Needs & Network Assessment- conducted by a licensed social worker

         

Tier 2: Health & Wellness Assessment- conducted by a registered nurse

Tier 3: Mental Health & Wellness- conduced by a licensed psychologist
